The Journey Home
Every person who comes to Transitions is on a different path. But the arc is always the same — from crisis, to stability, to a place of their own.
No two people arrive at Transitions the same way.
A veteran who served three tours and came home to find his support system gone. A 19-year-old who aged out of foster care with a trash bag of belongings and nowhere to go. A neighbor who lost a job, then an apartment, then everything else.
What they all share is that they found their way here — and that we were ready.
Engage: Meeting People Where They Are
The journey doesn't start when someone walks through our doors. For many, it starts on a street corner, under a bridge, or in an emergency room with nowhere to go next. Our job is to show up first.
Street Outreach: Our outreach team goes into the community to connect with people living unsheltered — building trust, offering resources, and creating a bridge to services. You can't help someone you can't reach.
Day Center: Open every day from 8:30 to 5:00, the Day Center is the heart of Transitions. Anyone 18 or older can walk in. Hot meals, showers, laundry, computers, clothing, case managers, and 50+ partner organizations are all here, all under one roof.
Emergency Housing: When someone is ready for a bed, we're ready to give them one. Our low-barrier emergency shelter offers up to 30 days of housing with no prerequisites — no sobriety requirement, no background check barrier. We accept everyone, because that's the only way this works.
Equip: Building Toward Independence
Shelter stabilizes. Services transform. Once someone has a safe place to sleep, the real work begins — and we meet every person with a plan built around their specific needs, challenges, and goals.
For most clients, this is the longest and most intensive part of the journey. It's where records get cleared, jobs get found, health gets addressed, and the pieces of a stable life start coming together — one case manager meeting, one program, one breakthrough at a time.
Veterans Program
In partnership with the VA and SCDVA, we provide dedicated beds and wraparound services for veterans navigating housing instability, mental health challenges, and the long road back from service. They served us. We show up for them.
Youth Program
For young adults ages 18–24 — many aging out of foster care with no family safety net — we provide housing, education support, job training, and the kind of consistent mentorship that helps them build a future on their own terms.
Actively Aging Program
Adults 55 and older face unique barriers — fixed incomes, physical limitations, and healthcare needs that require specialized support. This program provides housing and services designed specifically for them.
Homeless Court at Transitions
Outstanding fines and misdemeanor charges are one of the most common barriers to employment and housing. Homeless Court brings the legal process directly to our campus, helping clients clear their records and remove the obstacles standing between them and a job or a lease.
Contract Emergency Residential Services (CERS)
For individuals who need immediate, intensive support, CERS provides a structured residential environment with focused case management to stabilize and move people forward quickly.
Transitions Works
Our on-site career center connects clients with job training, resume support, mock interviews, and employment placement — because income is the bridge to independence.
Community Support Programs
Around 50 community organizations — mental health providers, medical clinics, addiction recovery programs, legal advocates, and more — come to Transitions to serve our clients. We provide the people and the space, they provide the expertise.
James’s Story
When James arrived, he entered our Men's Emergency program. Case manager Chiana helped him access healthcare, get his documentation in order, and apply for benefits. He landed his first job at a nearby McDonald's, started saving, and got connected to Homeless Court, where staff began working to resolve a suspended out-of-state license that had followed him for years — the kind of barrier that keeps people stuck, and the kind we exist to remove.
Transition: Moving into a Permanent Home
This is what everything builds toward. Not just getting off the street, but having a place that's yours, with the stability and support to stay there.
Rapid Rehousing: For clients who are ready, Rapid Rehousing connects them with permanent housing and provides short-term rental assistance and ongoing support to help them get — and stay — housed.
Monarch Manor: Our newest initiative, Monarch Manor is permanent supportive housing for those who need long-term stability to thrive — particularly older adults. It's the next chapter in what Transitions makes possible.
